YOUTH MENTORING PROGRAM
WELCOME PARENTS AND YOUTH!
The Jazzy Haven Project is a 6-week youth mentoring program that emphasizes growth and prosperity in the following areas: school, family, social life, spirituality, professional goals, and emotional wellness. The program is designed for youth ages 12-17. Sessions are held weekly and range from 1-2 hours per session. Sessions are available once per spring, summer, and fall term. Sessions are conducted in both an office setting and community outings.
Listed below is a QR code for potential mentees to complete an interest survey. The purpose of the survey is to allow mentees to share information about themselves with their mentors. Parents, please allow your child(ren) to complete the survey independently.
Please submit an interest survey FIRST! After submission, wait 24-48 hours to receive a welcome letter in your email. Once you have received the welcome letter, return to this page to submit the registration form and payment.

The 2023 Spring Session will begin on Sunday, January 22nd. Registration will open on Monday, January 2nd and will close on Friday, January 20th. Either scan the Registration QR Code OR click on the Registration Link to complete registration.
Welcome letters began going out on Tuesday, December 20, 2022.
There is still time to get interest surveys in before registration opens. Welcome letters will only be emailed to mentees who have submitted an interest survey. Registration will only be open to mentees who have received a welcome letter.
